Identifying gaps and risks, in preparation for developing a comprehensive improvement plan.


Components of the questionnaire:

  • Management and Organization: The presence of safety policies and procedures, organizational structure, responsibilities.
  • Physical environment: condition of equipment, ventilation, lighting, exits, hazardous materials.
  • Behaviors and Practices: Employees' commitment to safety procedures, use of protective equipment.
  • Emergency response: evacuation plans, training, availability of emergency equipment.
  • Training and Awareness: Training programs, awareness campaigns, knowledge assessment.
  • Documentation and Monitoring: Incident Records, Inspection Reports, Performance Indicators.
